Brake pads are thick blocks of friction material (think: very dense sandpaper) bolted into a caliper that straddles the rotor. When you press the brake pedal, hydraulic pressure pushes the pads into both faces of the spinning rotor. The friction slows the rotor β and the car. Pads wear down with every stop. Most pads have a thin metal wear indicator strip embedded near the bottom: when the pad wears thin enough, that strip scrapes the rotor and makes the classic high-pitched squeal that tells you replacement time has arrived.
